The grades, explained.
What your grade means in practice — what the gym reaches, what surgery addresses, and which tissue you’re actually carrying. Drawn, not photographed: real case photos return only as men explicitly consent to share them.
1Small, localisedyour range
A firm button of tissue right behind the areola — often just a puffy nipple. Hideable in most shirts, impossible to un-notice once you've seen it.
- The gym
- Leaning out sharpens everything around it, but the firm core doesn't shrink — it can even look more prominent on a leaner chest.
- Surgery
- The smallest procedure: gland excision alone through an edge-of-areola incision, often without lipo. Fast recovery, minimal scar.
2Moderate, visibleyour range
A clear mound that shows through a t-shirt — a mix of gland and fat, skin still tight. This is the most common presentation.
- The gym
- Training genuinely helps the fat share of it. The gland share stays — which is why months of clean effort often stall at 'better, but still there.'
- Surgery
- The standard combination: liposuction for the fat plus gland excision. Which is why the first question to any surgeon is whether they excise at all.
3Significant, skin stretchingyour range
Real volume with the skin envelope starting to stretch — the chest hangs rather than sits, and the nipple points down.
- The gym
- Weight loss can shrink the volume but not the envelope: stretched skin doesn't retract on its own, which changes what surgery has to do.
- Surgery
- Lipo + excision, and an honest conversation about skin. Some grade-3 chests get away without skin removal; many trade a longer scar for a flat result.
4Severe, breast-likeyour range
Feminising volume with significant skin excess — often after major weight change. Carrying this is heavy in every sense, and it's more common than the internet suggests.
- The gym
- Training is still worth it for everything else it gives you — but no amount of it reverses a stretched envelope with this much tissue behind it.
- Surgery
- Staged or combined excision with skin reduction, by a surgeon who does this regularly. This is exactly the case where volume of experience matters more than price.
↳ Which tissue is it?
The distinction you can’t feel from outside.
Gland is firm and fixed; fat is soft and diffuse. From the mirror they read the same — but everything downstream (the gym, lipo, excision) treats them differently. The photo check reads yours in seconds.
Glandular
Firm disc, small fat share. The gym can't reach it; excision removes it.
Pseudo (fatty)
Mostly fat, little to no gland. The one case where leaning out can genuinely resolve it.
Mixed
Both tissues together — the most common, and why 'just lipo' so often disappoints.
Skin excess
The volume went — the envelope stayed. A skin problem needs a skin answer.

What's actually in there
Two different tissues under the skin: soft fat, and a firm gland disc sitting directly behind the nipple. From outside they read as one mound — which is exactly why the wrong operation gets sold.
